What you can expect
Personal contribution
Keep in mind that the personal contribution starts when the indication has been granted. It also has to be paid during the stay at a temporary place. You can read more about this on the website of the CAK.
Options for temporary stays
The care mediator strives to find a temporary place within the current or preferred care organisation, if that is possible. If a temporary place is needed somewhere else, the care mediator will take care of this as well. Sometimes, there are multiple options. These will be discussed with you and your relative, so the best fit for the situation can be chosen. On the other hand, temporary stays are often rare, and there is not always a choice.
Care transfer
When a temporary stay has been found, the care mediator organises the transfer and will inform the parties involved. You do not have to do anything for the care transfer.
Making the temporary residence homely
Even though the stay is usually temporary, it helps if you help your relative make the residence feels as much like home as possible. You should also ask your relative about their stay, because if they feel at home, they can often times choose to keep living there.
Moving to the temporary place
It is important that you organise practical support, such as transport and moving personal items.
Call of the temporary residence
After your relative moves to the temporary residence, they stay on the waiting list at the preferred location. When there is a possibility of an available spot, you, your relative, or the care mediator will be contacted.
Vindt uw naaste het fijn op de overbruggingsplek?
Geef dit dan aan bij de zorgbemiddelaar, in veel gevallen is het mogelijk om op de overbruggingsplek te blijven wonen.
Tips
- Even though in principle you will be contacted when there is a spot at the preferred residence, it is helpful if you stay in touch. Contact the preferred location once every three months, to stay up to date on the waiting time.
- Moving to a new home, even if it is temporary, is a big step. We notice that people benefit a lot from (emotional) support from their relatives.
